Care Plan Coordinator
ROLE OVERVIEW
The Care Plan Coordinator assures that the long-term-care admission process, MDS/RAI process, restorative program, and care planning is completed accurately and timely for the state of South Dakota. The Care Plan Coordinator plans and implements Restorative programs in collaboration with the licensed therapist and Interdisciplinary Care Team to meet the physical and psychosocial needs of clients to maximize the client's existing abilities, emphasize independence instead of dependence, and minimize the negative effects of disability with an attitude of realist optimism. The incumbent is involved in providing restorative nursing for clients of all age groups and will supervise and educate restorative nursing aides. This position plays a critical role in 1) maximizing the highest potential reimbursement for the financial viability of the facility and community 2) development of the total plan of care for each resident and 3) the survey process, ensuring compliance with state and federal regulations. The role may include other responsibilities and duties as assigned based upon needs and requirements.

MINIMUM QUALIFICATIONS
• Education/Experience: Education commensurate with the requirements of the RN licensure. Experience in Long Term Care preferred.
