(CHW) Community Health Worker, FT
The Community Health Worker (CHW) is responsible for helping patients and their families access and navigate community services, other resources, and to help adopt healthy behaviors.

Essential functions of position include, but are not limited to:
• Be responsible for establishing trusting relationships with patients and their families, while providing general support and encouragement.
• Work closely with medical providers and other clinical personnel to ensure patients have comprehensive and coordinated care plans.
• Be knowledgeable of community, state and federal resources and provide referrals for services, etc. to agencies as appropriate.
• Act as a patient advocate and liaison between the patient/family and community/state/federal service agencies.
• Promote, maintain and improve the health of patients and their families by setting patient goals, ensuring patients attend appointments and holding patients accountable.
• Manage assigned caseload of patients.
• Maintain HIIPA compliance at all times.
• Arrange and/or attend staff meetings, trainings and other meetings as requested.
• Effective communicate and demonstrate the mission of Fall River Health Services.

CHW provides social support and informal counseling, advocates for individuals and community health needs, and demonstrates respect by ensuring the dignity of the patient and their family.
In addition to contact with patients, clinic providers, nursing staff and other in-clinic personnel, the CHW has frequent contact with other departments such as lab, and x-ray, and the business office. Externally the CHW community and state resources.

Education and Experience:
• Must have or be able to obtain Community Health Worker Certification.
• Be able to make decisions that will best address the needs of the patient and their family.
• Be able to keep patient and family information confidential due to the sensitivity of materials and information handled.
• Ability to make suggestions on workflow or system efficiency and effectiveness.
• Ability to prioritize.
• Ability to work independently and within a team environment.
• Have good organizational skills and be able to handle multiple priorities while remaining professional and calm.
• Have effective phone and computer skills.
• Ability to work at a high-volume while ensuring a high-level of accuracy
