Medical Records Clerk, FT
The Medical Records Clerk assures that patients’ clinical records are maintained according to established facility policies and procedures. Performs clerical duties that assist the multidisciplinary care team in assuring that individual resident clinical records are appropriately maintained. The Medical Records Clerk performs clerical duties for the Medical Records Department. The Medical Records Clerk assures that all records are maintained, completed, and preserved according to established policy and procedure, consulting with the HIM Manager as needed.

Education/Experience: High school diploma or equivalency.
Technical Skills: Prior medical records experience and medical terminology helpful. Strong organizational and customer service skills. Able to work independently. Demonstrates ability to type proficiently.
PC Competencies: Basic computer skills. Knowledge of Windows and EXCEL programs

The Medical Records Clerk:
Scans paper documents into the Electronic Medical Record
Validates that clinical records have the proper signatures and all the required documentation is completed accurately and appropriately.
Maintains format uniformity of forms, policies and procedures book and other records in the facility. This includes updating these items both manually and electronically.
Responds to Patient Health Information requests in a timely manner and in accord with HIPAA requirements; assures that PHI records are documented and input (scanned) into the electronic medical record (Epic) system in accordance with medical records policies and procedures.
Maintains individual clinical records from admission through discharge in the electronic medical record, maintains medical records supplies and answers phone
Scans or destroys old records in accord with medical records policies and procedures.
Assures all job functions and communications are carried out in a cooperative manner, encouraging a team spirit in and among departments. Demonstrates strong customer service skills internally and externally.
